At any of our seven outpatient clinics, we serve patients with chronic kidney disease (CKD) and acute kidney injury by providing the highest standard of treatment, including medication management, diet plans, and preparation for dialysis.

Management of chronic hypertension is crucial to maintaining kidney health. Our providers work with the patient and their primary care provider on managing blood pressure through medication, diet changes, and remote monitoring.

Home Dialysis
Once an eligible patient is determined to need dialysis, our providers work with the patient and our partners to provide training and equipment to allow the patient to perform dialysis at their home without the need for in-center treatment.

In-Center Dialysis
Our providers partner with Fresenius Medical Care, DaVita, and other dialysis centers to treat end-stage renal disease (ESRD) patients in a comfortable environment.

After you receive a kidney transplant, it is crucial to continue following up with a Nephrologist to ensure your care is monitored.
